As a top priority, Dr. Haak will reach out to you to schedule a free introductory consultation outlining how our unique services can address your specific needs. If we are a good fit for your family, we will then discuss the next steps and type of assessment required to move forward.
Intake paperwork is conveniently provided digitally. If required, a copy of your child’s insurance card and any additional requested paperwork will be required at this time.
An invoice will be sent requesting payment for the $150 registration fee.
We work with families with and without insurance.
If insurance is requested, we will verify ABA therapy coverage through your insurance provider. We will then schedule an assessment while awaiting insurance authorization. This usually takes around 48 hours but can take longer depending on your insurance carrier. If private pay is requested, we will simply schedule and complete the assessment.
Next, we will use the assessment to perform a final review of your child’s needs and make therapy recommendations accordingly.
